REMINDER –

For each new BCBSM group enrolled as a member you have two options:

1. Receive a $500.00 incentive for each 10+ member group signed up with MBPA or MFBA. Receive a $100.00 incentive for each member group with less than 10 employees that sign up with one of the Associations.

Qualifications for 10+ Membership Incentive Bonus:

• We must receive a copy of the groups completed membership application with dues payment AND a copy of the group’s completed Part C Agreement (Showing the number of employees) to qualify for the $500.00 incentive or special COBRA incentive.
